Dental Insurance:
Weekly Rates May 1, 2008 to April 30, 2009
Work 20-29 hours                  $3.70 single            $14.82 family
Work 30-40 hours                  $2.23 single            $11.49 family    

Vision Plan:
Employee Paid Supplemental Insurance Option
Available to employees who work 20 or more hours per week:
Low Option:
Single $1.20         Single + 1   $2.26                 Family   $3.31  

Medium Option:
Single  $1.33                         Single + 1    $2.51                Family   $3.89

Earned Time:
Accrual based on hours worked.  These days can be used for illness, vacations, personal day or cash out.  Available after 90 days retroactive to 1st day of employment.

                                1-5 years               accrue up to 20 days per year
                                5-10 years             accrue up to 28 days per year
                                10 + years             accrue up to 33 days per year
Holidays:
OHC recognizes 8 holidays: New Years Day, Presidents Day, Memorial Day, 4th of July, Labor Day, Columbus Day, Thanksgiving Day and Christmas Day.  Employees that work on the Holiday, receives time and one half their rate of pay.

Life Insurance:
Employees who work 30 or more hours per week receive a company paid $10,000 life insurance policy.

Tuition Reimbursement:
Up to 2 classes per year for Full time; 1 class per year for part time.  Available after 1 year of employment.

Pay in Lieu of Benefits:
Must work 24 or more hours per week to receive 15% more to base pay and waive the following benefits Medical, Dental, Life and Disability Insurances, Tuition Reimbursement, Earned Time, and Bereavement Leave.

Nursing Scholarship Program:
100% paid for C N A to LPN or LPN to RN.  Must be here for 1 year to apply.  Space is limited to 2 candidates per year for all of OHC.

Supplemental Insurance:
Short Term Disability, Cancer and Accident insurances available through Colonial insurance. Employee Paid through payroll deduction available to employees working 24 or more hours per week.

Retirement Program:
401k retirement program that allows salary deferral on a Pre-Tax basis.
All employees over the age of 21 are allowed to contribute.  OHC offers a matching contribution.

Employee Assistance Program
A free benefits and is available to all employees and/or immediate family members, living in the household. The service is a confidential.  EAP has been established with Family Services Central Massachusetts to provide counseling for employees and family members on a wide range of personal problems including: Marital, Parent/Child, Alcohol Abuse, Drug Abuse, Financial Difficulties, Stress/Anxiety and Depression.  EAP services include initial assessment, short-term counseling, referral and follow-up. Bereavement Leave  
For the purposes of attending a funeral or memorial service for an immediate family member, a paid absence of up to three days (8 hours per day is the maximum benefit) will be provided to you. 
A one day paid absence (scheduled work day) will be provided for attending the funeral or memorial service for the following relations grandparents, mother-in-law, father-in-law, aunts, uncles, step-relatives, brother-in-law, sister-in-law, and children-in-law, niece or nephew.

Family Medical leave
Available after 1 year of employment and must work 1, 250 in the prior year.   This time frame is based on a rolling calendar year.

Discounted Uniforms at the Uniform Outlet
Payroll deduction available after 3 months of employment.

Direct Deposit
Available to any bank

Jiffy Lube Discounted VIP Services
With the Jiffy Lube VIP Card, available at HR.
