Tara D’Andrea, MBA, NHA, FACHCA
Director of Operations, Oriol Health Care
Administrator, Holden Rehabilitation & Skilled Nursing Center
Tara D’Andrea serves as Director of Operations for Oriol Health Care and Administrator for Holden Rehabilitation & Skilled Nursing Center. In her role, she oversees Holden Rehabilitation’s skilled nursing and post-acute care services, ensuring a seamless continuum of care for residents.
With over 25 years of experience in healthcare, including 17 years as a Licensed Nursing Home Administrator, Tara is a dedicated leader in the skilled nursing and rehabilitation field. She holds a bachelor’s degree in Psychology from Worcester State University and an MBA with dual concentrations in Management and Healthcare Management from Fitchburg State University.
Tara is passionate about improving patient care, strengthening clinical programs, and ensuring Holden Rehabilitation continues its legacy of excellence in skilled nursing and rehabilitation.
Cynthia Snyder, RN
Director of Nursing Services at the Holden Rehabilitation and Skilled Nursing Center
With over 20 years of experience in skilled nursing and leadership, Cynthia Snyder is committed to enhancing patient care and safety at Holden Rehabilitation & Skilled Nursing Center.
Cynthia has a proven track record of leading facilities to 5-star CMS ratings, securing deficiency-free surveys, and earning National Service Excellence Awards for customer service and patient satisfaction. She has collaborated with area hospitals to strengthen care transitions and has played a key role in establishing Holden as a preferred provider in the region.
Cynthia is also an active participant in professional organizations, continuously working to develop new programs that enhance patient outcomes. She resides in Paxton, MA, with her husband and family.
Oprah Boateng, BSN, RN
Assistant Director of Nursing Services at the Holden Rehabilitation and Skilled Nursing Center
Oprah Boateng serves as Assistant Director of Nursing at Holden Rehabilitation & Skilled Nursing Center, where she supports clinical operations, staff development, regulatory compliance, and quality improvement initiatives to ensure the highest standards of resident-centered care.
With seven years of healthcare experience, Oprah began her career as a Certified Nursing Assistant before advancing to Registered Nurse and nursing leadership roles. She holds a Bachelor of Science in Nursing and is currently pursuing her Master of Science in Nursing, further strengthening her expertise in nursing administration and education.
Oprah is passionate about fostering a collaborative team culture and promoting best practices in clinical care. She is committed to continuous learning, mentoring nursing staff, and upholding Holden Rehabilitation’s tradition of excellence, safety, and compassionate care.
