Tara D’Andrea, MBA, NHA, FACHCA
Administrator
Tara D’Andrea serves as Director of Operations for Oriol Health Care and Administrator of Holden Rehabilitation & Skilled Nursing Center, where she leads clinical operations and supports the delivery of high-quality short-term rehabilitation, long-term care, and specialized respiratory services.
With more than 25 years of experience in healthcare, including 17 years as a Licensed Nursing Home Administrator, Tara is a respected leader in post-acute and long-term care. She holds a bachelor’s degree in Psychology from Worcester State University and an MBA with dual concentrations in Management and Healthcare Management from Fitchburg State University.
Tara leads with a focus on clinical excellence, team development, and a compassionate, personalized approach to care—ensuring every resident and family feels supported and respected. In 2026, she was nationally recognized by the American College of Health Care Administrators (ACHCA) as a Distinguished Nursing Home Administrator, reflecting her leadership and commitment to advancing quality care in skilled nursing.
Cynthia Snyder, RN
Director of Nursing Services
Cynthia Snyder serves as Director of Nursing Services at Holden Rehabilitation & Skilled Nursing Center, where she leads the nursing team in delivering high-quality, resident-centered care.
With more than 20 years of experience in skilled nursing and healthcare leadership, Cynthia brings a strong track record of clinical excellence and operational success. She has led facilities to achieve top-tier quality ratings, deficiency-free surveys, and national recognition for customer experience and patient satisfaction.
Cynthia is known for her collaborative leadership style and her commitment to strengthening clinical programs, supporting staff development, and enhancing patient safety. She works closely with area hospitals to improve care transitions and continues to advance outcomes through a focus on continuous improvement and compassionate care.
Oprah Boateng, BSN, RN
Asst. Director of Nursing Services
Oprah Boateng serves as Assistant Director of Nursing Services at Holden Rehabilitation & Skilled Nursing Center, where she supports clinical operations, staff development, regulatory compliance, and quality improvement initiatives.
She began her career as a Certified Nursing Assistant before advancing to Registered Nurse and nursing leadership roles, giving her a well-rounded perspective on patient care. Oprah holds a Bachelor of Science in Nursing and is currently pursuing her Master of Science in Nursing, further strengthening her expertise in nursing administration and education.
Oprah is passionate about fostering a supportive, team-oriented environment and promoting best practices in clinical care. She is committed to mentoring staff, encouraging continuous learning, and ensuring every resident receives care that is both clinically excellent and compassionate.
Our leadership team works closely with nursing, therapy, and support staff to ensure a coordinated and consistent approach to care. We prioritize clear communication, individualized care planning, and a strong team environment that supports both residents and those who care for them.
