Jennifer Crowley, NHA
Administrator
Jennifer Crowley, NHA, brings more than 20 years of experience in long-term care to her role as Administrator at Oakdale Rehabilitation & Skilled Nursing Center. Her career reflects a comprehensive understanding of skilled nursing and healthcare operations, having advanced through roles including Certified Nursing Assistant, Staff Scheduler, Medical Secretary, and Admissions Coordinator before becoming a licensed Nursing Home Administrator.
Jennifer holds a Bachelor of Science in Health Administration, completed her Administrator in Training program, and passed the NAB exam. She is recognized for her strong leadership, operational expertise, and commitment to delivering high-quality, resident-centered care.
Throughout her career, Jennifer has led facilities to achieve 5-star CMS ratings, deficiency-free survey results, and National Service Excellence Awards for customer service and patient satisfaction.
After previously serving as Administrator at Oakdale, Jennifer has returned to continue leading the center. We are pleased to welcome her back as she brings a continued focus on quality care, consistency, and a positive team environment that supports both residents and staff.
Heather Perry, BSN, RN

Heather Perry, BSN, RN, serves as Director of Nursing Services at Oakdale Rehabilitation & Skilled Nursing Center, where she leads the nursing team in delivering high-quality, resident-centered care.
Heather brings a strong clinical background and leadership experience in skilled nursing, with a focus on quality outcomes, regulatory excellence, and team development. She plays a key role in supporting nursing staff, strengthening clinical programs, and ensuring compliance with state and federal standards.
Heather is dedicated to fostering a collaborative, supportive environment where staff are empowered to provide compassionate, personalized care. Her leadership helps ensure that every resident at Oakdale receives care that reflects dignity, respect, and clinical excellence.
